When the CCC formed in 2006, we listed specific strategies for each of our initiatives. Here is the list of items we hoped to achieve as part of our 2009-2011 Strategic Plan - please note that a check next to the item means we have already met or exceeded that strategy:
Case Management (CM) Strategies
- To complete an assessment of healthcare CM resources in Wake County and use that information to build out the services component of CareScope by end of 3rd Q 2009.
- Hire a part-time non-clinical case manager in 1st Q 2009 to provide a low-level of case management to ensure that patients complete referrals. (Note: in 2011, we now have a full-time, bi-lingual Case Manager)
- Hire a part-time clinical case manger by 3rd Q 2009 to ensure that patient's clinical linkages are met. (Note: our nurse case manager is full-time)
- Train CCC providers on standards of care utilized by Community Care of North Carolina.
IT Strategies
- To have 100% of partner organizations using CS enrollment and eligibility.
- To have 100% of partner organizations using CS to make Project Access referrals.
- To get a pharmacy assistance program (PAP) functional by end of 3rd Q 2009.
- To have services built out in CS by end of 3rd Q 2009.
- To get one interface with a partner organization up and running in 2010. (Note: as of summer 2011, we have three partner interfaces feeding CS.)
Pharmacy Strategies
- To create a pharmacy workgroup to determine ways to reduce medication costs and increase medication access across the partner organizations.
- Implement at least two enhancements or new initiatives related to pharmacy.
Community Assessment Strategies
- To complete a capacity assessment by end of 2nd Q 2009, which would include information on primary care, specialty care, urgent care and pharmacy.
- To participate in the WCHS Community Assessment, scheduled in 2010.
Model of Care Strategies
- Engage the Clinical Committee to define urgent care.
- Develop a workhroup to assess the need for urgent care resources in Wake County.
- If neessary, develop a plan for the implementation of urgent care resources in Wake County.
Advocacy Strategy
- Create an Advocacy Committee and determine the agenda.
