The CCC operates the following initiatives under the auspices of the Wake County Medical Society:
Community Case Management
The primary purpose of the Community Case Manager (CM) is to assist in assessing, implementing, coordinating, monitoring and evaluating the options and services needed to meet the needs of uninsured patients with ongoing medical conditions and/or needs. The CM receives referrals from hospitals for patients who are uninsured and do not have a primary care home, have a chronic condition and use the hospital emergency departments for non-emergency care. The case manager focuses on connection to primary care and patient education to manage diseases and reduce ED visits. The belief is that this personalized follow-up approach will result in stabilizing people with the chronic illnesses that often lead to emergency room visits, and help get these individuals on the path to a healthier life.
IT Initiative
The CCC has obtained a community health information system, CareScope, that provides enrollment and eligibity, community referral and other functions for the CCC partner organizations. The CCC has integrated another software, the MedData Prescription Assistance Program, with CareScope to automate the PAP application process for interested community partners.
Project Access
Wake County's physician community is compassionate and caring. Since 2000, over 500 physicians and clinical staff have continued to provide free care to uninsured men, women and children through Project Access. Qualified patients may be referred into the program due to medical necessity. Patients are carefully screened for eligibility before being referred. SOAR Program
Individuals who are eligible for the Federal Social Security Disability Program and the Supplemental Security Insurance program but are homeless, or are re-entering the community after spending time in institutional settings (hospitals, jails or prisons), or are dealing with substance abuse, mental health issues or cognitive disabilities find the program application process daunting. The SOAR program is an outreach and application effort, to locate individuals who may qualify for benefits and assist them with the application process. The goal is to expedite disability benefits approval for qualified applicants so that they can access income, health insurance, housing and needed services. Referrals come from hospitals, shelters, clinics, and other agencies serving the homeless. Obtain information on the North Carolina Coalition to End Homelessness here.
Wake Crisis Cooperative
The vision of the Wake Crisis Cooperative is to improve the health of persons with Mental Health/Substance Abuse issues by providing crisis care in a timely manner and in the most appropriate setting. Its mission is to enhance countywide resources and improve Mental Health and Substance Abuse system processes to ensure that the care provided to individuals in crisis is provided by a coordinated and collaborative system of public and private providers.
